Do not include PHI or patient information.
Do not include patient or resident names, medical record numbers, health details, or other information that could identify a patient or resident.
What anonymous means here
This form does not ask for your name, email address, phone number, employer, or account login. Norra does not include your network address in the delivered report. As with any website, Norra's hosting and email providers may process ordinary network metadata needed to operate and protect their services.
Report details are delivered only to Norra's designated security and compliance recipient. Do not use this channel for emergencies.
